E SQUARE Template®
for over 5 years. The templates are organized in the same way that
Emergency Physicians process their Histories and Physicals in their heads.
They progress from the specifics of the Present Illness to the Review of
Systems, Past Medical History, Physical Exam, and Differential Diagnosis.
The really great advantage of the templates is that they optimize your
documentation and minimize your writing time.
The other advantage is risk
management. The templates remind you what to document with each patient so
that the most relevant points pertaining to that patient’s chief complaint
don’t get missed. They even include pertinent items that JCAHO looks at such
as reassessment of pain, patient education, etc.
I would strongly recommend the
E SQUARE Template® for any Emergency Department.

During my 15 years as an Emergency Medicine physician I have used many
documentation tools including dictation and templates. None of these
provided me with the ease and completeness of the
E SQUARE Template®.
In these days of increasing medical legal risk I look to protect
myself as much as possible while maintaining a good work flow. It is simple
