The Program Overview

The purpose of the E SQUARE Physician Scribe® program is to promote optimal patient care by returning the physician to the patient. In addition, it is designed to improve the quality of the work environment for all members of the ED medical team. A Scribe is a person who is responsible, mature, and possesses a basic knowledge of medical terminology and techniques. The Scribe must also demonstrate excellent people skills, the ability to work in a team setting and the stamina to work in a stressful and fast paced environment.

The Scribe works as an assistant to the physician and is an essential member of the ED team. The presence of a Scribe on the ED team will expedite patient flow through the ED, decrease the time a patient will wait to see a doctor, increase the time the doctor can spend with the patient, and decrease the overall time a patient must spend in the ED.

There are 4 overall objectives of the E SQUARE Physician Scribe® program :

  1. Increase physician direct patient care.
  2. Enhance patient satisfaction.
  3. Improve documentation.
  4. Improve the working environment in the ED for all members.

What a Scribe Does

The Scribe accompanies the physician on patient assessments in order to gather and transcribe the history and physical data for the patient. The Scribe also records any procedures performed and diagnostic tests ordered. The Scribe completes referral forms, retrieves and collates test results, flags test results for the physician, and prints discharge orders. The Scribe ensures that the chart is ready for physician review and presents the chart for the physician to sign. The Scribe also ensures that all the prescriptions and relevant orders are signed. The Scribe also reviews each chart to verify that all the necessary data has been recorded to meet the appropriate coding level for each patient.

The Training

Each Scribe will take a screening test to identify basic needed skills. Scribe candidates who pass the screening test will then participate in an intensive three day in-class training program called the Scribe Academy. This curriculum based, interactive program teaches the various tools and techniques for functioning in the clinical environment as a Physician Scribe. After successful completion of the Scribe Academy, the Scribe will receive closely supervised ED clinical training in the Emergency Department. The ED clinical training is designed to provide each Scribe with a varied set of clinical experiences within the ED.
